Care Homes and Nursing Homes fulfil an important role in providing care for the elderly in Wiltshire. Wiltshire CCG and its partner organisations are able to support you in a number of ways.

Medicines Optimisation in Care Homes (MOCH)

The MOCH Team are a team of pharmacists and pharmacy technician teams who work across BANES, Swindon, and Wiltshire providing support and advice around medicine use in care homes: wccg.moch@nhs.net

Salisbury NHS Foundation Trust

A Trusted Assessor has recently started a pilot project at Salisbury Foundation Trust. Working on behalf of local nursing and residential homes, the Trusted Assessor aims to act as an independent and unbiased assessor to support and facilitate effective and timely discharges from acute hospital settings into care homes. The Wiltshire Care Partnership “represent[s] and support[s] independent providers of residential, nursing and domiciliary care for older people and adults with disabilities in Wiltshire, working alongside commissioners to achieve the provision of high quality, safe services”.
