A Trusted Assessor has recently started a pilot project at Salisbury Foundation Trust. Working on behalf of local nursing and residential homes, the Trusted Assessor aims to act as an independent and unbiased assessor to support and facilitate effective and timely discharges from acute hospital settings into care homes.
