End of life care – Treatment Escalation Plan (TEP) resources

In December 2014, a new Treatment Escalation Plan (TEP) was launched in Wiltshire to:

  • improve the experience of patients, their families and carers
  • ensure the wishes of patients, their families and carers are communicated between health providers

We developed the TEP as part of the multi-agency end of life programme which involved patient representatives, hospital and hospice staff and GPs. We’re supporting the TEP implementation with an education programme for health professionals and information for patients and their families and carers.

Patients can discuss their TEP with health professionals at any stage. The TEP can also be altered to mirror the changing wishes of patients.
